The District Health Centre Express is a primary healthcare initiative of the Government. Through public-private partnership, medical-social collaboration, it provides district-based primary healthcare services aiming to:
- Enhance public awareness of disease prevention and their capability in self-management of health;
- Drive healthy lifestyle for prevention of chronic disease;
- Support the chronically ill to prevent deterioration;
- Enhance members’ access to primary healthcare service.
The core station will offer primary healthcare services through individual service sessions and group intervention. It will provide primary, secondary and tertiary prevention programmes. The satellite stations will offer primary prevention programmes and other group activities.
The core team of District Health Centre Express consists of nurses, a physiotherapist, an occupational therapist, a dietitian, a pharmacist, and social workers. Operator of the District Health Centre Express also enlists the help of private healthcare service providers in the district, such as western medical practitioners, Chinese medicine practitioners, optometrists, podiatrists, etc.., to form a district health network.
An individual who:
- is a holder of the Hong Kong Identity Card issued under the Registration of Persons Ordinance (Cap. 177, the Laws of Hong Kong), except those who obtained their Hong Kong Identity Cards by virtue of a previous permission to land or remain in Hong Kong granted to him and such permission has expired or ceased to be valid; or a child who is a Hong Kong resident and under 11 years of age; and
- agrees to enrol in the electronic Health Record Sharing System (eHRSS); and
- is a resident or working in the district of the corresponding District Health Centre.
He/she should present their identity document and residential address proof.
Joining the District Health Centre Express Scheme is voluntary. Those who wish to withdraw from the Scheme should inform staff of the corresponding District Health Centre Express.
Registration and membership are free. The membership is lifelong.
The Sai Kung DHCE Scheme includes primary, secondary, and tertiary prevention programmes. Primary prevention consists of health promotion and disease prevention activities; secondary prevention includes health risk assessments and screening for diabetes and hypertension; tertiary prevention addresses chronic disease management, low back pain, and osteoarthritic (OA) knee pain.
Yes, healthcare service sessions of the District Health Centre Express are referred based on clinical needs, and the maximum number of subsidized sessions of different Schemes are set.
Under the Scheme, the District Health Centre Express members can receive subsidised healthcare professional services for the targeted diseases/scope. Individual members may receive additional service(s) or treatments at their own expenses.
